Reaching Every Adult and Child's Home
REACH provides in-home and community-based supervision and education for individuals with disabilities to give families or care providers time off from their considerable responsibilities. REACH provides individuals with experiences that educate and enlighten to help children and adults live more independently and fulfilled.
Family Support and Community Integration and Habilitation Wavier
Residential Habilitation and Support Services are designed to ensure the health, safety, and welfare of the individual. Services and supports help with the acquisition, retention or improvement of skills needed to live successfully at home. This includes personal care, meals, shopping, money management, and other activities of daily living.
Community Habilitation, Individual or Group Services are provided outside of the home that support learning and assistance in the areas of self-care, socialization, communication, relationships, and other daily living skills. This can be provided on a one on one basis or in a group setting.
Participant Assistance and Care Services are provided to allow individuals to remain and live successfully at home and participate in their communities. Services support individuals with activities of daily living, self-care, and mobility with hands on assistance, prompting, reminders and supervision.
RESPITE services are provided on a short-term basis to provide temporary relief to the primary caregivers. Service allows for the assistance with daily living activities, at home or in the community. Respite cannot be used to allow the caregiver to go to work or attend classes. Respite can also be provided through Care Giver Support.
Aging & Disabled and Traumatic Brain Injury Waivers
Attendant Care services are provided to meet the daily living needs at home and in the community. Assistance is provided to help with activities of daily living, grooming and hygiene, meal preparation, mobility and household chores.
Homemaker services are provided to assist the consumer with household tasks so they can remain in a clean, safe and healthy home. Activities may include housekeeping tasks such as dusting, sweeping, laundry, and doing dishes, meal planning and preparation, running errands.
Respite allows for short-term temporary relief of the primary caregiver.
Residential-Based Habilitation Assistance is offered to individuals on the TBI Waiver only. Service allows for assistance with skill acquisition, retention or improvement. Assistance is provided with personal care, meals, shopping, errands, chores, and leisure skills.
REACH provides a trained care provider who will provide activities to help your loved one live a more fulfilled life. REACH also gives you a respite and a chance to take a break.
